Richmond Behavioral Health is seeking a full-time REACH Home Coordiantor. The selected candidate will perform intermediate skilled human support work, performing clinical and coordination services as part of the REACH Home (REACH CTH or Transition Home), conducting coordination services for clients with serious and persistent mental illnesses and co-occurring disorders to assist them in maintaining safety and overall stability in community living, providing support assistance and advocacy in maintaining client's financial, housing, medical, psychiatric and social needs, and related work as apparent or assigned. Works with individual, REACH staff, and system of support to coordinate weekly treatment team meetings, discharge planning, and communicating recommendations; facilitates treatment team meeting participation. Work is performed under the general direction of the REACH Home Program Manager (Adult CTH / ATH) OR REACH Program Manager (Child CTH) based on site.
Essential Functions

  • Implements REACH team support for individuals with intellectual and developmental disabilities and intense behavior needs.
  • Model therapeutic groups and implements therapeutic curriculums at the REACH Home for REACH staff.
  • Maintain communication and relationships with community partners.
  • Complete Waiver Residential Crisis Supports assessments and reassessments for individuals at the REACH Home.
  • Develop and revise treatment plans for individuals admitted to the assigned REACH site (residential).
  • Review and/or develop safety plans for each REACH admission; revise as needed during the person's admission.
  • Ensures the coordination of treatment team meetings each week and facilitate meeting.
  • Participate weekly in Multi-Disciplinary Team (MDT) meeting and provide clinical updates on all current guests.
  • Ensure MDT recommendations are incorporated into current treatment delivery and discharge planning / discharge instructions.
  • Possess strong case conceptualization skills
  • Complete Crisis Education Prevention Plans (CEPPs) as needed.
  • Participate in required meetings with REACH Home Team and Supervisors.
  • Identify and coordinate necessary training and technical assistance needs for REACH staff, community providers, natural supports, and other components of the system of care to help prepare them for the REACH Home guest's step-down back to the community.
  • Link individuals to additional supports and services as recommended.
  • Complete REACH-sponsored/required trainings on an ongoing basis.
  • Travel to off-site appointments/meetings when necessary.
  • Complete documentation in the electronic health record in accordance with collaborative documentation, Licensing, DMAS, and agency requirements.
  • Participate in the leadership on-call rotation to include providing crisis support/coverage to the REACH CTH or Transition Home and/or working shifts as needed (days, nights, weekends and holiday rotation).
  • Ensures required trainings are current and completed in a timely manner.
  • Performs other related work duties as assigned. Position Requirements

    Knowledge, Skills and Abilities
    General knowledge of the principles and practices of providing care to the emotionally disturbed, socially dysfunctional, behaviorally disordered, mentally ill and/or substance abusive clients and their families; general knowledge of community resources and human services network; general knowledge of community-based behavioral health principles and psychotherapeutic treatment techniques; general knowledge of the theories and techniques of counseling and crisis intervention; thorough knowledge of intake techniques and procedures; ability to evaluate and screen clients; ability to prepare adequate records and to report findings and data effectively; ability to solve problems within scope of responsibility; ability to establish and maintain effective working relationships with clients, associates, service providers and the general public.
    Education and Experience
    • Bachelor's degree and or Master's degree prefered, with coursework in counseling, social work, psychology or related field and moderate experience working with intellectual disabilities, developmental disabilities and mental health, or equivalent combination of education and experience.
    • Must meet QDDP and QMHP-A criteria
    • One year of experience in I/DD, including programming for individuals with significant behavioral needs
    • Current Virginia license as an LPC, LCSW, LPC, or LMFT, LSATP or license eligible preferred Special Requirements
      • Clinic and community work required.
      • Valid driver's license in the Commonwealth of Virginia.
      • CPR/First Aid/AED, Safety Care, and Person-Centered certifications.
      • Medication Administration and Gastrostomy Tube Certifications
      • Ability to work flexible hours to include evenings, weekends, and holidays if needed.
      • Participates in supervisory on-call schedule as designated for the REACH CTH and Transitional Homes
      • Qualified clinicians are eligible for a sign-on bonus as defined by the terms and conditions of the approved sign-on bonus policy. Full-Time/Part-Time

        Starting Salary Range

        $57,293.01, rate also includes 6.25% diff. which is offered to essential staff.
